The DDD Provider Search allows users to search for providers. You may filter your search results by name, service, location (county, zip code), language, acuity, and residential licensure setting or use a combination of searches and filters to browse provider options.

Support Coordinator Information
What is the Role of a Support Coordinator:
· Is the primary point of contact—or “go-to” person—for the individual/family
· Help connect the individual with services and other resources in the community
· Is available 24/7 for emergent situations, and can schedule other interactions with the individual/family at their convenience
