Who Qualifies for DDD Services?

To receive DDD-funded services in New Jersey, an individual must meet both Medicaid and DDD eligibility requirements.

Basic Eligibility Criteria:

• Be a New Jersey resident
• Be 21 years of age or older (applications may begin at 18)
• Be Medicaid-eligible and maintain that eligibility
• Meet the functional criteria for having a developmental disability

Functional Criteria:

An individual must have a chronic physical and/or intellectual disability that:

1. Appeared before the age of 22
2. Is lifelong in nature
3. Substantially limits functioning in at least three of the following areas:

How to Apply for DDD Services

To begin the process, individuals must submit a DDD Intake Application, available on the NJ Department of Human Services website. The application packet is available in English and Spanish.

Application Requirements:

• Proof of identity and age
• Proof of New Jersey residency
• Documentation of disability (medical records, evaluations, etc.)
• Medicaid eligibility documentation
