Introduction

Hudson County Transcend provides specialized transportation for Seniors (60 and over), handicapped and disabled residents of Hudson County. Most vehicles are specially equipped for persons using wheelchairs.

Transcend Will Provide:

	Non-emergency transportation to medical, mental health, dental, hospital, employment, education, and recreational sites as schedules permit.
	Curb to curb service
	Service is on a first com first served basis

Responsibilities of the Rider:

	Call a minimum of five (7) working days in advance to request a ride.
	Try to make your individual appointment between 9:30 a.m. and 3:00 p.m.
	Be on time to meet the vehicles at your designated time and place. The vehicle will wait no longer than 15 minutes.
	If you must cancel your scheduled trip, call the agency by the morning of your appointment.
	Three or more uncancelled appointments and you will be temporarily suspended for future service.
	You must wear seat belts
	NO SMOKING on the vehicle
	Please be aware that causing a disturbance or not observing the above Transcend rules may result in discontinuance of service.

	Name Disability (necessitate lift?)
	Social Security/Medicaid Number
	Phone Number; Date of birth
	Do you have a seeing eye dog?
	Any other pertinent information
