Camden County Senior Citizens Community Services Of Camden
County, Inc. 1. INTRODUCTION This guide was written to enable you, the client, to understand how to use the various services offered by the Sen-Han Transit system. Also, eligibility limitations of SEN-HAN Transit services are discussed. Please keep this handy as your guide for the SEN-HAN Transit system. We have included a section in the back of this guide to give you information on local municipal bus operations. For information on various programs offered by NJ Transit for senior citizens and disabled residents of New Jersey, call 1-800-772-3606. You can now reach the New Jersey Telecommunications Relay Service (NJ TRS) 24 hours, every day simply by dialing 7-1-1. SEN-HAN Transit is the outgrowth of efforts by the Camden County Board of Chosen Freeholders to provide coordinated special transportation services to its elderly and disabled citizens. In 1979, the Camden County Division of Senior Services, along with an ad hoc committee of the County's Human Services providers, recognized the need for coordination to provide the elderly and disabled with efficient, sensitive, and cost-effective transportation. The County's current provider, Senior Citizens United Community Services (SCUCS), initiated the concept of coordination under the guidance of the Camden County Division of Senior Services in 1980. In subsequent years, the Camden County Board of Chosen Freeholder designated SCUCS to operate a rural transportation program and in 1984 designated SCUCS as operators of the county's expanded transportation initiative, made possible through State Casino Revenue funds. The SEN-HAN system also provides transportation services through direct contracts with several municipalities and human service agencies. SEN-HAN TRANSIT SCUCS has maximized the resources of its various funding sources in order to provide an effective, cost-efficient service to all programs and their cients. In addition to the special services provided by SCUCS, Camden County's elderly and disabled can obtain transportation from a number of local municipalities, as well as NJ Transit's bus and rail lines. The Camden County Office of Consumer Services for the Disabled is responsible for the planning monitoring of the Casino-funded services and is also focusing its efforts on the development of a county-wide coordinated delivery system. Additionally, the Camden County Board of Chosen Freeholders has appointed a Citizen's Advisory Committee to provide Guidance and recommendations to the Board on transportation needs and services. Post-secondary education transportation is provided to disabled students attending local colleges and universities. Post-secondary education transportation is provided currently to the Blackwood and Camden City campuses of Camden County College, Rowan University and Rutgers University. Who? This service may be used by clients referred by the schools or themselves to SEN-Han Transit. Where? Students are transported on a group basis by SEN-HAN Transit. How? Disabled students under 60 years of age should
call SEN-HAN SPONSORED EMPLOYMENT TRANSPORTATION Employment transportation is provided to a variety of job training sites and sheltered workshops in Camden County. Who? This is a modified fixed-route service and the operators of the projects refer riders to SEN-HAN Transit. Where? Transportation is currently provided on a "space available" basis to the Occupational Training Center (OTC) and the Abilities Center. How? You would arrange your ride through the staff of the project currently employing you. You will be notified of a pick-up time, location, and return time. Disabled individuals under 60 years of age can be certified through their employment project. This service provides door-to-door transportation for senior citizen volunteers registered with the Retired Senior Volunteer Program (RSVP). Who? Transportation may be used by any volunteer member of the RSVP referred to SEN-HAN Transit by the volunteer office. Where? Transportation is provided to the entire Camden County area served by RSVP. How? Seniors citizens interested in participating in the program should call the RSVP office at (856) 663-4773 to obtain information about volunteer assignments. Volunteers are encouraged to provide their own transportation whenever possible, as RSVP has limited transportation available. Transportation is available for personal business requests within Camden County on a pre-reserved basis. This service may either be provided on an individual basis or grouped with another client going to the same location. What? Clients may request transportation for a variety of personal needs. Examples are home/hospital visits, banking, social security office, various agencies, social events, et. Destinations must be in the Camden County area. When? Transportation is available on a first come, first serve basis between 9:00 am and 3:00 pm, Monday through Friday and on a very limited basis on Saturdays. How? To request a personal business ride you would call SEN-HAN at (856) 456-3344 between 9:00 am and 4:00 pm three (3) days before you need to be transported. If the demand for this service increases, a regional system would be instituted. Please remember, you will probably be transported with another client going to the same city as you have requested. This client may be a medical client so you must be ready for your home pick-up an hour before your pick-up time. You would then be required to be ready to come home at a set time as well. Transportation is provided daily to the Lakeland Medical Day Care Program in Camden County through contract with the project. SEN-HAN supplements the transportation provided. by the center. This door-to-door service and the operator of the medical day care program refers riders to SEN-HAN Transit. What? The Lakeland Medical Day Care Center provides nursing services, medical supervision meals, and other services to clients with specific medical conditions. Who? This service may be utilized by clients referred by the program to SEN-HAN Transit for transportation. Disabled clients may have their application completed by the project and forwarded to SEN-HAN Transit for certification. How? Call the center directly to inquire regarding their eligibility requirements, availability of space and transportation assignments. You may reach the Lakeland Medical Day Care Center by calling (856) 374-6720. Daily transportation is provided to clients living within close geographical proximity to certain nutrition sites and social day programs in the county. Where? Clients are transported to three sites operated by the Camden County Nutrition Program of the Salvation Army. The two Camden City locations re Salvation Army and North Camden Elders Center. The county location is at the Pine Hill Nutrition Center. Also served are the Neighborhood Assistance Program, Audubon Park; the Lakeland Social Day Program, Blackwood; and the Merchantville Nutrition site. How? Clients desiring service should contact the managers of the sites to register for a ride. You must call the centers to inquire as to their eligibility guidelines.
This service offers group recreational transport to Camden County organizations, agencies, and municipalities whose clients are senior citizens and/or disabled. A fee for service is assessed equal to the expenses incurred by SEN-HAN to provide the group's ride. How? Your request for transport must be made in writing to the Operations Manger, SEN-HAN Transit, 146 Black Horse Pike, Mt. Ephraim, NJ 08059-2035 at least two weeks before the event. Please utilize your group's letterhead and include in your correspondence: the date of the event, origin and destination, time schedule, contact person's name and phone number, approximate number of passengers, and special need requirements (i.e. wheelchair lift, etc.). You will receive confirmation of your request and the reimbursement cost required to provide the service. This reimbursement fee may either be paid by the organization or by the individuals involved in the activity. This choice is up to the group requesting service. Payment in full by one check must be received by CSUCS one week prior to the planned event. Please make your check payable to "SCUCS, Inc." and send it to the attention of the Operations Manager. Non-emergency medical transportation provides door-to-door rides to doctor's offices, hospitals, and other medical facilities. Who? This service may be used by any resident of Camden County who is 60 year of age or older, or a permanently disabled individual (subject to a physician's certification). Where? Non-emergency medical rides are currently available anywhere in Camden County, to borderline facilities in connecting counties, and limited hours/days in center city Philadelphia (call for locations). How? Clients needing this service should call the central intake unit at (856) 456-3344 between 9:00 am and 4:00 pm, Monday through Friday. Advance reservation is required and ride appointments should be made (between two weeks and three days) in advance. Disabled residents may call (856) 456-3344 between 9:00 am and 4:00 pm, Monday through Friday, to request a Disabled Transportation Application for disabled certification. Hearing impaired clients with TTD equipment may ccall NJ Relay TTY & TDD at 1-800-852-7899 (Voice only 1-800-852-7897 to request assistance contacting SEN-HAN Transit. Confirmation All clients with ride appointments are to call (856) 742-8937 between 5:00 am and 10:00 am the working day before their ride appointment to confirm they still plan on going. Medical rides not confirmed by 10:00 am the working day before your appointment will be cancelled. This policy must be strictly enforce as 8 to 10 individuals request rides each working day that cannot be honored, and 5 to 8 individuals do not show up for appointments. To enhance the possibility of ride sharing, passengers are asked to be ready on (1) hour before their scheduled appointment. Passengers in high-rise buildings are expected to be in the lobby area when the van arrives. Rides are available on a first come, first served basis with the available rides booking quickly. Schedule ahead to secure your ride! Clients requiring an escort to accompany them to their doctor's appointments must notify the intake clerk so space will be reserved for both individuals. Please inform the clerk of your special medical transport needs: if you require a wheelchair lift vehicle: if you use a walker or cane; if you have any other disability we should be aware of when planning your ride. The SEN-HAN system operates a weekly fixed-route food shopping service and bi-monthly mall shopping service in 18 municipalities. Route stops are modified or changes as requests are received from the riders with service provided to local supermarkets and shopping centers. Who? This service may be used by any resident of Camden County 60 years of age or older, permanently or temporarily disabled individuals (subject to a physician's certification), or residents of any age residing in Waterford Township and Winslow Township. Where? Weekly service is provided in the following municipalities: Audubon Borough, Audubon Park, Barrinton, Chesilhurst, Clementon, Collingswood, Gibbsboro, Gloucester City, Haddonfield, HiNella, Somerdale, Stratford, Magnolia, Merchantville, Mt. Ephraim, Waterfor Township, Winslow Township, and Woodlynne. How? Call (856) 456-3344 to inquire about the current stops in your municipality and asked to have a shopping flyer mailed to you. Upon boarding the vehicle, you will receive an Intake Form from the driver. Fill in the form and return to the driver. The driver will assist you with your packages but you are asked to limit your purchases to what you can carry home due to space limitations. Transportation is also provided by some municipalities who operate their own buses. These vehicles offer transportation for shopping and recreational purposes. In cooperation with the Camden County Office of Consumer Services for the Disabled, the Camden County Board of Chosen Freeholders and local municipal funds, fifteen municipalities directly provide bus transportation to their senior citizens and disabled residents. Shopping Veterans non-emergency medial transport is available to clients in need of transportation on a first come, first served basis, depending on ride availability. Who? Eligible clients are: Veterans having served a minimum of 90 days in active military service of the United State and having a discharge other than dishonorable, or if the active military service was less than 90 days, client must have received a medical discharge. Where? The rides are provided as "group" transport when possible. Transportation destinations include: VA facilities (i.e. county clinics, regional offices including trips to NJDMACA - Division of Veteran's Loans, Grants & Services (DVLGS) District Offices), Veteran's Hospital, 38th & Woodlynne, Philadelphia, PA. How? Client eligibility and certification will be determined by the staff of the local VA district office. Inquiries for certification should be made to: NJ Department of Military Veterans Affairs, 251 North Delsea Drive, Suite F, Deptford, NJ 08096, (856) 853-4186. This project provides limited transportation for Camden County individuals employed by private sector companies within the county. Who? Service is available to senior citizens or disabled residents. Disabled residents must complete the SEN-HAN Transit Certification form. Call (856) 456-3344 for Handicap Transportation Application. What? During SEN-HAN's normal working hours, such request will be handled on a space available basis. Requests for rides before and after regular hours may be provided if space is available. Please ask to speak to the Operations Manager for special consideration. The system will endeavor to connect the passenger to public transportation. The fee for service is based on a zone formula and passengers are charged for each zone traveled. Passengers who use public transit for part of their ride receive a 50% discount on their zone passes. Passengers are billed monthly and will be charged for any rides not cancelled. Cancellation calls may be made Monday through Friday between 6:30 am and 3:00 pm by calling (856) 456-3344. How? Individuals requiring this service are to call the SEN-HAN Transit Operations Manager at (856) 456-3344 to make arrangements for this service. SEN-HAN Transit, in conjunction with Gloucester and Cumberland County Para-transits, offers a non-emergency medical shuttle service to Philadelphia on Mondays, Tuesdays, and Wednesdays. Sen-Han Transit will pick up its passengers at their homes, transport them to the shuttle transfer point (Gloucester Inn), and transport our passengers to their homes. To reserve a seat, you must call SEN-HAN Transit between three (3) days and two (2) weeks in advance of your appointment. PHILADELPHIA SHUTTLE
* Gloucester and Cumberland County only SEN-HAN TRANSIT TRANSPORTATION POLICIES OVERVIEW This section covers a brief overview of SEN-HAN Transit policies approved by the Advisory Committee. A complete package of these policies may be obtained by calling SEN-HAN Transit at (856) 456-3344. To expand the capacity of the system, SEN-HAN will encourage and offer linkage to public transportation systems. As situations warrant, passengers will be assisted in securing any available discount programs on these systems. Passengers refusing to accept linkage can be denied service. For the purpose of SEN-HAN Transit, "disabled" is defined as a client whose condition precludes their use of public transportation. SEN-HAN Transit has instituted a voluntary donation policy. The suggested donation is $1.00 per ride. Passengers who are eligible clients of a third party reimbursement system must provide SEN-HAN with this information. These may include: Medicaid, Division of Developmental Disabilities (DDD), Division of Vocational Rehabilitation (DVR), Veterans Affairs (VA), auto insurance accident claims, and private rehabilitation companies. Failure to provide this information may result in denial of services. Conditions warranting denial of service include: refusal to wear seatbelt, failure to confirm rides, passengers who are a threat to the safety of others or under the influence of drugs or alcohol, those who are in precarious health, and those who are incontinent of bowel or bladder.
