Transition Requirements Checklist

The following checklist corresponds to specific sections of the individuals with Disabilities Education Act requirements of 1997 related to the transitions requirements .

Participants in the meetings

When the purpose of the IEP meeting is the consideration
of transition services: ------------------------------------------------------Yes---- No

1. Did the public agency invite the student?

 

 

2. Did the public agency invite a representative of any other agency that is likely to be responsible for providing or paying for transition services?

 

 

3. If the student did not attend the IEP meeting did the public agency take steps to ensure that the student's preferences and interests were considered in the development of the IEP?

 

 

4. Were the efforts documented to ensure that the students preferences and interest were considered?

 

 

5. If an agency invited to send a representative to a meeting did not do so, did the public agency take other steps to involve their participation in the planning of any transition services?

 

 

6. Were the steps documented?

 

 

Parent and student participation

1. If the student is any age (but at least 14 years of age and older) was parent notice provided that transition will be discussed and does the notice?

2. Indicate that one of the purposes of the meeting will be the development of a statement of transition service needs or consideration of transition services?

 

 

3. Indicate that the public agency will invite the student?

5. Indicate the time and location of the meeting and who will be invited?

 

 

6. Indicate to both the parent and the student at least one year prior to the student turning 18, that all parental rights transfer to the student at 18 years of age?

 

 


Content of individualized education program
